North Carolina CNA Requirements and State Approved CNA Training Programs

According to the Bureau of Labor Statistics, there are more than 321,000 Certified Nurse Assistants employed at facilities across the United States. These skilled caregivers dedicate their time working for senior citizens, ensuring they get the help they deserve. It’s nothing short of amazing the type of tasks nurse aides perform for the patients they serve. The small army of nurse aides is expected to receive unprecedented growth in the next decade as demands for senior care increases. Due to the nature of the job, the federal government set guidelines for the training and certification of nurse aides across the nation. The North Carolina Department of Health and Human Services, uphold federal regulations by the development of a Nurse Aide Training and Competency Evaluation Program and management of an up-to-date Nurse Aide Registry.

Certification Requirements

In order to improve the quality of care in nursing homes and adhere to federal regulations, North Carolina requires all nurse aides to obtain certification before volunteering or working for compensation at state-licensed nursing facilities. The purpose of training and certification is to ensure nurse aide candidates understand federal and state standards and can competently and safely perform their duties as entry level employees.

While the state requires certification for all nurse aides before employment, there is an allowance for nurse aide students. An employer can hire a nurse aide in training for a period of 120 days while he/she is enrolled in an approved training program. The student must complete training, testing, and certification during this period in order to continue working as the period is not renewable. It is the employers’ responsibility to verify a prospective employee’s status on the registry before hire.

If you’re an out-of-state CNA, foreign nurse, military-trained CNA, or nursing student, you can work as a CNA after taking the nurse aide competency evaluation for placement on the registry. Contact the NC DHSR to find out if you qualify for exemption from training.

Prerequisites to Training

One of the best incentives for becoming a CNA for many is the fact that you don’t need a high school diploma to train. Federal authorities do not stipulate minimum education requirements, but some training providers may ask for proof of high school completion or GED. Other schools may administer an entrance exam to determine your eligibility for admittance or request ACT or SAT scores– it all comes down to the schools’ individual requirements. Having a high school diploma (or not) affects your chances of employment with some employers. It is also a factor if you hope to advance to other careers in the health industry.

Following are some other prerequisites to training:

  • 17 years or older
  • High school diploma or GED (recommended)
  • Drug testing
  • Negative TB test
  • FBI criminal background check – contact the NC DHSR for a list of disqualifying felonies
  • Doctor’s clearance that you have no communicable diseases and are physically fit to handle the rigors of the job
  • Immunization records, including varicella, Hepatitis B, TDaP, and two-step PPD

North Carolina Certified Nurse Aide Training Requirements

North Carolina is one of the few remaining states that do not require nurse aides to take a training program before challenging the competency evaluation. However, training is recommended as it will provide ample opportunity to obtain knowledge and learn the skills necessary to pass the exam. State approved CNA I programs offer a minimum of 75 hours of instruction, including 16 hours of hands-on training at an approved nursing facility. Another benefit of opting to enroll in training is the opportunity to practice for the competency evaluation to improve your chances of success. If you fail the competency exam three times, you must enroll in a training program before you can challenge the test again.

If you don’t have any knowledge or experience in the healthcare industry, you’ll benefit from classroom instructions and live demonstrations. Course content includes basic nursing principles, feeding and personal care, transferring patients, taking vital signs, observation and charting, medical terminology, medical equipment, infection control, safety and emergency, patients’ rights, law and ethics. You’ll also learn other soft skills such as communication with patients, their family members and the healthcare staff along with job search and job interview procedures.

Cost of CNA Training North Carolina

The cost of CNA training in North Carolina is much lower than other states. Still, the average cost of $400 for tuition may seem insurmountable if you don’t have the money or a source of funding. Consider your training costs as an investment in your future, and while there may not be options for financial aid upfront, you can apply for reimbursement of your expenses if you get a job within 12 months of certification.

High school students, nursing students, and nursing home employees have a distinct advantage when it comes to training. If you’re enrolled in high school, you can study to earn Nurse Aide I certification through Health Occupations Education programs.

Nursing students can obtain a certificate of completion of Nurse Aide I training. If you’re enrolled in a state-accredited nursing degree program, you can ask your dean about nurse aide certification made available by a memorandum between the governing bodies.

Many North Carolina nursing homes are eligible to provide training to their employees. If you’re employed at a nursing home in another capacity, you can ask your employer to sponsor your training. Nursing homes are an ideal place to train as you’ll not only have the opportunity to get free training and paid practicum, but also the possibility of employment immediately following certification.

Nurse Aide Testing

The NC DHSR has contracted with Pearson VUE to administer the state nurse aide evaluation and report results to the Nurse Aide I Registry. Your training provider will, in most cases, submit your application for testing. However, you can also apply online for immediate scheduling at a test center of your choice. If you’ve graduated from a state-approved program, you must take and pass the test within 24 months of graduating.

The nurse aide evaluation is a two-part exam that tests your competency to work in a real-world setting. There is a written and a skills component. You can request the oral exam in place of the written component if you have difficulty reading. The oral exam is available in both English and Spanish. First-time testers must register to take both components on the same day.

The written exam contains 70 multiple choice questions. The oral option contains 60 multiple choice questions and 10 reading comprehension questions. Sample questions are available in Pearson Vue’s Candidate Handbook. All requests for oral testing must be submitted at the time of application.

For the skills evaluation, you’ll have to demonstrate five skills, selected at random, in 30 minutes. The Nurse Aide Evaluator will rate your performance of the skills. You must perform all five skills correctly to pass the exam. The candidate handbook provides a full listing of skills.

Certification Costs

  • Written and skills exam – $101
  • Oral and skills exam – $101
  • Written retest – $24
  • Skills retest – $77
  • Oral retest – $24

Walk with the following items on exam day:

  • Two forms of ID (one must contain a current photo)
  • Three sharpened No. 2 pencils and eraser
  • Watch with a second hand
  • Uniform scrubs and flat, non-skid, closed-toe shoes for the clinical

Out-of-State CNAs

Since North Carolina does not practice reciprocity or transfer of CNA licenses with other states, the process of transferring your license is not as simple as other states. To obtain placement on the state’s nurse aide registry to be eligible for work, you’ll need to:

  • Pass a state-approved nurse aide training and competency testing for Nurse Aide I or
  • Pass the state nurse aide evaluation administered by Pearson Vue.

Renewal

Nurse Aide I listings remain valid for 24 months (2 years) from the day you pass the competency test. You can renew your license for another 24 months as long as you report your qualifying employment to the registry. Your reported qualifying employment must be for paid work, which you provided under the supervision of a Registered Nurse for a minimum of 8 hours – with the 24-month period.

The registry will mail the renewal application 3 months before expiration. It is your responsibility to inform the registry of address or name changes and to ensure your certification is renewed before it expires. If your license expires, you must retest/retrain to renew.
